Each survey link is only valid for one response; respondent is only eligible to receive one gift card per survey response. Gift card supplies are limited and not all survey respondents may receive a gift card if gift card supplies run out. Respondents must complete the entire survey to be eligible to receive a gift card. Gift card will be issued in U.S. dollars. Employees of AmerisourceBergen and/or its affiliates shall not be eligible to receive a gift card. For the purposes herein, the term “relative” shall mean spouse, partner, parent, child or sibling. Gift card recipient specifically acknowledges that gift card recipient shall be responsible for the payment of all tax, title fees, license fees, and other charges as may be required under any law, ordinance, statute, regulation or otherwise. Gift cards are not transferable nor exchangeable and cannot be exchanged for money. By accepting the gift card, each entrant agrees to release, discharge and hold harmless AmerisourceBergen, its legal representatives, affiliates, subsidiaries, agencies and their respective officers, directors, employees, and agents from any damages whatsoever suffered, sustained, or allegedly sustained in connection with the survey or acceptance of the gift card. Gift cards are subject to additional terms and conditions through AmerisourceBergen’s vendor partner. AmerisourceBergen reserves the right to alter these terms and conditions at any time and in its sole discretion. These terms and conditions and any matters relating hereto shall be governed by and construed in accordance with the laws of the state of Pennsylvania and jurisdiction over any and all disputes shall be exclusive to the state and federal courts in Montgomery County, Pennsylvania.
